We are in network with Blue Cross Blue Shield, Aetna, United Healthcare, Optum, and pending in network with MedCost. If you have insurance through those companies, please provide your insurance information when scheduling your initial appointment. Co payments are due in full one day prior to your scheduled appointment.

If we are not in network with your insurance carrier, the listed fees below are due in full one day prior to your scheduled appointment and we can provide a superbill if requested. Please check your out-of-network coverage with your insurance for reimbursement information.

15 minute phone call discussing your needs to determine if New Moon Psychiatry is the most therapeutic practice for you. No cost for this phone consult. Required before scheduling initial appointment. 

Initial Visit $300.00

60 minute evaluation for new patients. Provider approval required for telehealth appointment. Release of information required for New Moon Psychiatry to request previous treatment records prior to appointment. Fee due in full one day prior to your scheduled appointment.

Hospital Discharge Initial Visit $375.00

60 minute evaluation for new patients following acute psychiatric hospitalization. Referral by hospital staff required. A release of information will be required by the discharging facility to discuss referral. A release of information may also be required for New Moon Psychiatry to request hospital records if needed. Schedule appointment by phone within 48 hours of discharge. Provider approval required for telehealth appointment. Fee due in full one day prior to your scheduled appointment.

20 minute phone call for non urgent needs. Available to established patients who have completed both an initial evaluation and the initial follow up appointment. This appointment is meant for unexpected needs in between scheduled follow ups and does not take the place of pending scheduled follow up appointments. Fee due in full one day prior to your scheduled appointment. 

Established Patient Follow Up $150.00

30 minute follow up appointment for maintenance treatment. Available to established patients who have completed both an initial evaluation and the initial follow up appointment. Telehealth option available. Fee due in full one day prior to your scheduled appointment.

Established Patient Add On- Printing Medical Records

Printing of requested medical records requires a fee of $0.50/page per copy. Fee due in full one day prior to printing.
